RELEASE, WAIVER OF LIABILITY, AND BOOKING POLICY AGREEMENT
RELATING TO SERVICES OFFERED BY LIGHT MANOR AND GREGORY K. CADOTTE.
PLEASE READ THIS AGREEMENT CAREFULLY. BY SIGNING, YOU ACKNOWLEDGE THAT YOU HAVE READ, UNDERSTOOD, AND AGREE TO THE TERMS BELOW. THIS AGREEMENT INCLUDES IMPORTANT INFORMATION ABOUT LIABILITY, RISKS, AND POLICIES REGARDING SERVICES PROVIDED BY THE COMPANY.
SECTION 1: RELEASE AND WAIVER OF LIABILITY
1.1 Acknowledgement of Risks
The Company offers various services, including but not limited to Meditation training, Hypnosis, Hypnotherapy, This Life Regression, Past Life Regression, Between Life Regression, and other energy-based or movement-based therapies (“Services”). Participation in these Services may involve inherent risks, including but not limited to:
• Physical discomfort, such as muscle soreness, dizziness, or fatigue.
• Emotional release, such as crying, anger, or vulnerability.
• Exacerbation of undisclosed or unknown conditions.
I acknowledge that these risks cannot be entirely eliminated and knowingly accept them.
1.2 Purpose of Services
The Services provided by the Company are complementary wellness practices and are not intended to replace medical, psychological, or therapeutic care from licensed professionals. The Company does not diagnose, treat, or cure medical conditions.
1.3 My Responsibility to Disclose
I agree to disclose any known medical conditions, injuries, psychological concerns, or medications that may affect my ability to participate safely in the Services.
1.4 Consent to Services
I understand that some Services may involve physical touch or addressing areas of the body. I consent to such practices and acknowledge my right to decline any aspect of the Services at any time.
1.5 Release of Liability
In consideration of my participation in the Services, I voluntarily:
• Release and discharge Light Manor, The Dancing Buddha, Gregory K. Cadotte, its officers, directors, employees, contractors, agents, affiliates, and successors (“Releasees”) from any and all claims, demands, or causes of action arising out of or relating to my participation in the Services, including claims arising from negligence.
• Assume all risks, both known and unknown, related to the Services.
• Indemnify and hold harmless the Releasees from any claims, costs, or damages arising from my participation.

Terms and Conditions
The hypnotherapy process is a partnership. We will work with your subconscious mind, yet the intentions you set, the preparation you make, and the actions you take are critical to achieving your best outcomes. The more you put in, the more you get out.
As your hypnotherapist, I have a responsibility to guide you through a great process. You have a personal responsibility to follow that process with diligence and commitment.
The 3 Fundamental Prerequisites

Submit every Assignment or Intake Form at least 48 hours before scheduling any session.

Be ready to join each session at least 5 minutes before the scheduled time.

Focus on following the given instructions precisely – nothing more, nothing less.
Failure to meet these prerequisites may result in the inability to proceed with therapy or training.
Policy for Missed or Rescheduled Sessions
Unconfirmed Sessions

Cancelling Sessions: If an intake form is not received at least 24 hours before the initial session, the session will be automatically canceled.

Rescheduling Sessions: Canceled sessions can be rescheduled at no cost once a timely Intake Form or Assignment is submitted.

Confirming/Postponing: Sessions are reconfirmed by an automatic reminder 24 hours in advance. If you need to postpone, provide at least 24 hours' notice.
Confirmed Sessions: Less Than 24-Hour Postponement or No Show

No-Show: If a session is booked and confirmed but not attended or rescheduled with less than 24 hours’ notice, it is considered a no-show.

Free Initial Session No-Show: No additional free initial session will be offered. To continue, a $100 fee will be required to book another intake session.

Paid Session No-Show: You will need to schedule another session and pay an additional $100 fee.
Showing up on time is an essential quality for success in hypnotherapy. Lack of professionalism and punctuality can affect not just your sessions but also your progress in therapy.
Policy for Scheduled Sessions & Changes

Canceling or Rescheduling:

With 48+ hours’ notice: $0.

With 24+ hours’ notice: $50.

With less than 24 hours’ notice: $100.

To continue sessions, a payment of $50 or $100 will be required.
Policy for Terminating Sessions
Sessions are offered as part of a structured program. They are not obligated to be given or continued under all circumstances.

Purchased Sessions, Attended None: Do not purchase sessions before completing the Intake Form and having an initial interview. If therapy is deemed inappropriate, a refund will be issued minus a $50 administrative fee.

Purchased Sessions, Attended One: If, during the first session, it is determined that therapy is not appropriate due to new information or inability to follow the process, a refund will be provided for remaining sessions minus $100.

Client-Initiated Termination: If you decide to discontinue after attending one session, any potential refund is at the discretion of Light Manor Hypnotherapy and may be less than the pro-rata value of the remaining sessions.

Therapy requires commitment, and incomplete engagement may be counterproductive. To ensure the best outcomes, clients are encouraged to complete their full program.
Legal Disclaimer
By scheduling a session with Gregory K. Cadotte at Light Manor Hypnotherapy, you acknowledge and agree that:

Gregory K. Cadotte is not a medical professional, attorney, or financial advisor and does not provide medical, legal, or tax advice.

Hypnotherapy sessions are intended for personal insight and transformation but do not substitute for professional medical or psychological treatment.

You are responsible for your own decisions and actions.

Sessions are for individuals 18 years and older.

Light Manor Hypnotherapy is not responsible for any damages, including personal injury, stress, or financial loss, resulting from the use of services or information provided.

Terms and Conditions Updates
These terms may be updated periodically. Continued use of services indicates acceptance of the most recent terms.

SECTION 2: BOOKING POLICY
2.1 Payment Policy
• Full payment is required at the time of booking to confirm your session. Payment secures your spot and ensures that the time is reserved exclusively for you.
2.2 Communication
• All booking changes must be communicated via email or the booking platform.
SECTION 3: TERMS OF PARTICIPATION
3.1 Voluntary Participation
Participation in the Services is entirely voluntary. I understand that I have the right to pause or stop any session at any time.
3.2 Confidentiality
The Company will maintain confidentiality regarding all personal information shared during sessions, in accordance with ethical and legal standards.
3.3 No Guarantees
I understand that results from the Services are subjective and individualized. The Company makes no guarantees regarding specific outcomes.
3.4 Governing Law and Dispute Resolution
This Agreement shall be governed by the laws of the Province of Ontario. Any disputes arising under this Agreement shall be resolved through mediation before resorting to litigation.
3.5 Severability
If any part of this Agreement is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
SECTION 4: CONSENT AND SIGNATURE
By signing below, I confirm that:

• I voluntarily accept all terms and conditions herein.
